Does this issue occur when all extensions are disabled?: Yes/No

As an user, I explicitly asked for xcodebuildmcp server but only saw "VS Code MCP Gateway", I have no idea what it is and it doesn't give confidence that the agent is picking up the right agent. The tool permission confirmation is also talking about the wrong MCP server.
